Yourself/Company ("User") Phase 2 Solutions, LLC ("Provider) |
| As an account User, you agree to be billed on a monthly basis for services and technology provided on www.forwardmyflyer.com by Phase 2 Solutions, LLC, a Pennsylvania Limited Liability Company. User will receive server space allocated on www.forwardmyflyer.com use for storing and managing consumer data. User retains the rights to the data processed through their account. User also agrees to not hold Provider liable for any circumstance resulting from or use of submitted data. User agrees that ownership of the technology provided through www.forwardmyflyer.com belongs to Provider and User is paying for usage of the technology. User will only be allowed one offer set change per month. An offer set refers to the two designs use to represent offers for both the sendor and receiver from the "Forward to a Friend" form designated for User on allocated server space. An offer set change is a design edit, re-design, or new design for sender and receiver offers. If design requests take more than one hour to complete, Provider has the right to charge $70/hour for each additional hour required to complete for design work. Provider will notify User before hand if additional time is needed to complete a design set. Provider will provide customer service and support services for User. User agrees that they will be billed on a monthly basis using the billing information provided on www.forwardmyflyer.com/payment. User agrees to receive services and to be billed for a minimum of 3 months from the date of online registration. If user decides to cancel within 3 months of this agreement, User agrees to be billed a $100 cancellation fee. After 3 months from the date of this agreement the terms of cancellation will become inactive and User may cancel at anytime without penalty. | |
